California Last Will and Testament – Sample Template

Parties Involved:

Testator: Alice Johnson
Address: 789 Oak Street, Los Angeles, CA 90001

Distributions of Property:

I hereby direct that my estate, including all real and personal property, be distributed as specified herein. This includes my residence at 789 Oak Street, Los Angeles, CA, and all other assets in my name at the time of my death.

Executor:

I nominate and appoint John Davis of 456 Maple Avenue, Los Angeles, CA 90002, as the Executor of my estate. If he is unable or unwilling to serve, I appoint Jane Smith of 123 Pine Road, Los Angeles, CA 90003, as successor Executor.

Guardian for Minors:

Should I be the parent of minor children at my death, I nominate Emily Brown of 321 Birch Lane, Los Angeles, CA 90004, as guardian of my children.

Specific Bequests:
